No! you can’t copy the same affiliate disclosure from other site like 10hotdeals.com, you should do some changes according to your requirements and and services that you are using.

But it’s always better to have a dedicated page for some of them. A parent page with links to these pages is a good idea as well.
Yes, you can simply add Disclaimer & affiliate Disclosure under same page. Also, as per the affiliate site policies you have to show your affiliate disclaimer in every page possible.
